Which authority licenses the facility

A facility in the emirate may be licensed within Sharjah or federally, depending on which authority issued the licence. That is not a detail to infer from a website: the licence says which authority it comes from, and that authority is where a question about the clinic, or a complaint about it, belongs.

The same two checks apply as anywhere in the country. The facility holds a licence, and each practitioner holds one of their own, which is where the permitted scope of practice is recorded.

Asking the insurer and the clinic billing office the same question, in writing, is the reliable route. Prices for refractive surgery are set per clinic rather than per emirate, so a lower figure in one emirate is a statement about inclusions rather than about geography.
